Position Purpose:   

The RN/Case Manager (Pulmonary Hypertension) provides ongoing support and expertise for patients with Pulmonary Arterial Hypertension (PAH) through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs.

Duties and Responsibilities:

  1. Performs and coordinates initial assessments and ongoing reassessments of the patient’s status; for new pulmonary hypertension (PH) evaluations assists PH specialist with evaluation and coordination of care for required procedures to determine if patient has PAH; for established PAH patients  includes patients that are on specific PAH vasodilators, coordinates care and medical therapy, including oxygen needs, rehabilitation, labs and PAH/heart failure therapy; documents patient case information within the electronic medical record (EMR); performs chart review/audit.
  2. Coordinates care with PH specialist for inpatients with PAH; coordinates response to therapy with PH specialist and primary team; attends to PAH medication needs, including pre-authorization, education, coordination of care with OSU pharmacy and specialty PH pharmacy; coordinates care to avoid PAH therapy interruptions or dosing errors at discharge; provides PAH education for inpatient rehabilitation facilities; assists those with primary discharge responsibilities with the coordination of PAH medication to ensure there is no interruption.
  3. Coordinates scheduling and appropriate anticoagulation before catheterization procedures with cath lab personnel; attends PH cath lab cases and provides patient education about PAH therapy as needed.

  4. Ensures that the patient’s medical needs are addressed; consults with the patient’s physicians as needed, coordinating plans of treatment, and advocating for the patient when necessary.

  5. Participates in case conferences by providing information pertinent to patient’s needs/goals; attends PHs meetings that include coordination of care with different medical specialties and/or pharmacy for updates in PAH patients’ clinical care and PH related meetings to improve service for patients and educational needs for staff (i.e. discussion of difficult cases, education about new medications or resources available for patients)

  1. Partners with the Medical Director in development and review of the patient’s individualized coordination of care plan.
  2. Serves as patient advocate; identifies and assists patient in accessing information; empowers patients in decision making for care planning where appropriate; enhances a collaborative relationship to maximize the patient’s and family’s ability to make informed decisions.
  3. Fosters intra-facility and inter-facility working relationships to help accomplish goals. Acts as a liaison between primary care providers, specialist, and/or patient.
  4. Maintains accurate and timely patient information; assist in data collection for reporting/funding sources.
  5. Promotes understanding of the medical factors affecting the targeted population.
  6. Participate in outreach activities to the entire target population, as directed.
  7. Recommend program/service changes to meet gaps in service in the community.
  8. Participates in Quality Assurance review activities, as directed.
  9. Performs other work related duties as required.


  • Requires a Degree in Nursing
  • Requires at least two years of nursing experience
  • Must possess current Ohio Registered Nurse license.
  • CPR certified required
  • Skill in performing clinical and medical procedures.
  • Knowledge of clinic and inpatient operations. Pulmonary hypertension experience preferred.


  • Prior case management experience preferred.
