Position Purpose:   

The Patient Navigator (PN) will work with CarePoint East General Internal Medicine Practice as part of the clinical care team focusing on the Medicaid patient population. This PN will be available to navigate any patient in need, with a primary focus on patients with a history of no show or cancellations to previous appointments. This focus will help to increase adherence with recommended follow-up and treatments in Medicaid patients seen in clinic. The Patient Navigator will assist patients and his/her family by identifying resources for them as they navigate through medical, insurance, financial and social issues.  He/She will address patient calls and inquiries through actively listening to patient’s concerns and requests. The PN will provide patient follow-up and will create and maintain patient profiles.  The PN will match patient needs with resources, assisting the patient in seeking available services and programs. The PN will help patients develop a follow-up plan to ensure that current and future needs are addressed. The PN will maintain appropriate tracking and reporting systems.  Additional related duties will be assigned.

Duties and Responsibilities:


  • Works as a liaison between physician offices, specialty clinics, and patients to meet the needs of patients scheduled for an clinic appointments and procedures across the OSU Wexner Medical Center.

  • Work closely with primary care physician and care team to ensure that patients have coordinated care.

  • Enters and retrieves necessary patient information using the Medical Information system (EPIC)

  • Functions as a liaison with all hospital departments to facilitate accurate and prompt implementation of patient care.

  • Works effectively as a team member and maintains a positive relationship and open communication with other team members and hospital departments.

  • Anticipates the needs of all customers (internal and external) and assumes responsibility for solving customer problems when appropriate.

  • Coordinates the collection of information for patient care documentation; maintains patient chart information.

  • Contacts patient prior to appointments to confirm appointment and identify barriers or concerns that may interfere with their preparation and arrival to their scheduled appointment.

  • Carries out responsibilities with compassion and sensitivity to patients, families, visitors, and other staff. Is courteous to others and promotes mutual respect.

  • Collates department statistics as requested and forwards to management as directed.

  • Provide referrals for services to community agencies as appropriate.

  • Utilizes supplies and time (both own and others time) in a cost effective manner; systematically organizes and prioritizes work.

  • Contributes to performance improvement of the hospital, the clinic, and patient care team.

  • Assist with data collection, report generation and data tracking.

  • Other duties as assigned.

  • High school diploma.

  • Previous medical office or customer service experience.

  • Must be knowledgeable with medical office operations and systems; familiar with guidelines regarding patient confidentiality issues.

  • Must have excellent public relations skills and organizational skills.

  • Must have personal skills to deal with patients, physicians, and staff of all age ranges.

  • Demonstrates the capability to use office equipment (computer, facsimile, telephones, pagers, etc.) associated with job responsibilities.
  • Must have working knowledge of MS Office platforms such as (Excel, OneNote, Word, Powerpoint)


Bachelor’s degree in social work, psychology, public health, education and or other Health & Human Services is preferred.
